02.Home Health Agency (HHA)
Our licensed Home Health Agency delivers skilled, medical-based care in the comfort of home. Each care plan is supervised by nursing professionals to support health, safety, and recovery. Services include: Skilled Nursing Visits (RN/LPN) Medication Setup and Administration Wound Care and Disease Management Post-Surgical and Rehabilitation Support Therapy Coordination (PT, OT, ST) Chronic Condition Monitoring Health Assessments and Care Plan Oversight Transition and Hospital-to-Home Care

Beyond daily care, we offer supportive programs that strengthen families, promote wellness, and connect clients to essential community resources. 04.Structured Family Caregiving
Structured Family Caregiving is a Medicaid program designed to support family caregivers in eligible states by providing them with a tax-free stipend. It's a meaningful way to ensure that families can maintain their connections while receiving the support they need.
